Aims: This study aimed to investigate whether monocyte dysregulation and hyperinflammation serve as predictive markers for mortality in young patients with SARS-CoV-2 severe pneumonia. Methods: A prospective cohort study was conducted in a tertiary-level public University Hospital in Colombia. Forty young adults (18-50 years of age) with severe pneumonia and SARS-CoV-2 infection confirmed by qPCR, were enrroled. Serum cytokines and the monocyte phenotype profile, including PDL1/HLA-DR expression, were determined during the first 24 h of hospitalization. Routine laboratory parameters were measured throughout patient follow-up until either death or hospital discharge. We also included a cohort of twenty-five healthy control subjects. Key findings: Elevated levels of IL-10, IL-8, and IL-6 cytokines emerged as robust predictors of mortality in young adults with severe pneumonia due to SARS-CoV-2 infected. A descriptive analysis revealed a cumulative mortality rate of 30 % in unvaccinated and ICU-admitted patients. Patients who died had significantly lower expression of HLA-DR on their classical monocytes subsets (CD14+CD16-) than survivors and healthy controls. Lower expression of HLA-DR was associated with greater clinical severity (APACHE≥12) and bacterial coinfection (relative risk 2.5 95%CI [1.18-5.74]). Notably, the expression of HLA-DR in 27.5 % of CD14+/CD16- monocytes was associated with a significantly lower probability of survival. Significance: The early reduction in HLA-DR expression within classical monocytes emerged as an independent predictor of mortality, irrespective of comorbidities. Together with PD-L1 expression and cytokine alterations, these findings support the notion that monocyte immunosuppression plays a fundamental role in the pathogenesis and mortality of young patients infected with SARS-CoV-2. These findings hold significant implications for risk assessment and therapeutic strategies in managing critically ill young adults with SARS-CoV-2 infection.

Resumen Introducción: Las enfermedades cardiovasculares son la primera causa de muerte en el mundo. Su incidencia depende de factores de riesgo, los cuales pueden ser evaluados mediante escalas de riesgo cardiovascular que, de acuerdo con este, permiten establecer estrategias de prevención. Sin embargo, están dirigidas principalmente a áreas urbanas de Colombia donde se ha estudiado la carga de la enfermedad, y muy poco a las áreas rurales. Objetivo: Describir las características clínicas de una población rural que asiste a un programa rural de riesgo cardiovascular. Método: Estudio transversal en 540 participantes de un programa de riesgo cardiovascular de nivel primario. Se obtuvieron los datos sociodemográficos, clínicos y bioquímicos. Se estimó el riesgo cardiovascular mediante la escala de Framingham. Resultados: La hipertensión arterial fue el factor de riesgo de mayor prevalencia entre los participantes (95.5%); de ellos, el 70% presentaba un control adecuado de las cifras de presión arterial. En segundo lugar, se encontró sobrepeso/obesidad, con una proporción mayor en mujeres que en hombres (56.4% vs. 70.7%, p = 0.001). El tabaquismo fue de muy baja prevalencia (0.09%), al igual que las complicaciones en órgano blanco (hombres 8% vs. mujeres 2.2%, p = 0.001). El 75% de la población (n = 409) se estratificó como riesgo bajo de acuerdo con la escala de Framingham. Conclusiones: En esta población, la hipertensión arterial, el sobrepeso/obesidad y la coexistencia de diabetes mellitus tipo 2 tienen una prevalencia similar a la de otras poblaciones rurales y urbanas. El estrato de bajo riesgo cardiovascular agrupa a la mayoría de individuos y el tabaquismo es de muy baja prevalencia.


Abstract Introduction: Cardiovascular diseases are the leading cause of death in the world. Its presentation depends on risk factors, which can be evaluated using cardiovascular risk scores and according to its risk establish preventive strategies. However, these strategies have been mainly addressed to urban areas, where the burden of disease has been studied, and not so much to rural areas of Colombia. Objective: To describe the clinical characteristics of a rural population that attends a rural cardiovascular risk program. Method: It was performed a cross-sectional study in 540 participants in a primary-level cardiovascular risk program. Sociodemographic, clinical, and biochemical data were collected. Cardiovascular risk was estimated using the Framingham score. Results: High blood pressure was the highest risk factor in prevalence among the participants (95.5%), of whom 70% had adequate blood pressure values. Overweight/obesity was the second more prevalent risk factor, with a higher proportion in women respect to men (56.4 vs. 70.7%, p = 0.001). Smoking had a low prevalence (0.09%), as well as target organ dysfunction (men 8% vs. women 2.2%, p = 0.001). The 75% of the population (n = 409) was classified in low risk, according to the Framingham score. Conclusions: In this population hypertension, overweight/obesity and the coexistence of type 2 diabetes mellitus had similar prevalence respect to other rural and urban populations. The lower cardiovascular risk stratum grouped most of the participants and smoking had a very low prevalence.

Abstract Hemorrhagic shock and its complications are a major cause of death among trauma patients. The management of hemorrhagic shock using a damage control resuscitation strategy has been shown to decrease mortality and improve patient outcomes. One of the components of damage control resuscitation is hemostatic resuscitation, which involves the replacement of lost blood volume with components such as packed red blood cells, fresh frozen plasma, cryoprecipitate, and platelets in a 1:1:1:1 ratio. However, this is a strategy that is not applicable in many parts of Latin America and other low-and-middle-income countries throughout the world, where there is a lack of well-equipped blood banks and an insufficient availability of blood products. To overcome these barriers, we propose the use of cold fresh whole blood for hemostatic resuscitation in exsanguinating patients. Over 6 years of experience in Ecuador has shown that resuscitation with cold fresh whole blood has similar outcomes and a similar safety profile compared to resuscitation with hemocomponents. Whole blood confers many advantages over component therapy including, but not limited to the transfusion of blood with a physiologic ratio of components, ease of transport and transfusion, less volume of anticoagulants and additives transfused to the patient, and exposure to fewer donors. Whole blood is a tool with reemerging potential that can be implemented in civilian trauma centers with optimal results and less technical demand.


Resumen El choque hemorrágico y sus complicaciones son la principal causa de muerte en los pacientes con trauma. La resucitación en control de daños ha demostrado una disminución en la mortalidad y mejoría en el manejo del paciente. La resucitación hemostática consiste en la recuperación del volumen con hemoderivados como glóbulos rojos, plasma, crioprecipitado y plaquetas, en proporciones de 1:1:1:1. Sin embargo, esta demanda de hemo componentes podría no aplicarse para toda Latinoamérica u otros países de medianos y bajos ingresos. Las principales barreras para la implementación de esta estrategia serían la escasa disponibilidad de bancos de sangre y de hemoderivados insuficientes para contar con un protocolo de transfusión masiva. Una propuesta para superar estas barreras es el uso de sangre total fresca fría para la resucitación hemostática de los pacientes exsanguinados. Ecuador ha sido pionero en la implementación de esta estrategia con una experiencia ya de seis años, en que han demostrado que la sangre total tiene ventajas sobre la terapia de hemo componentes incluyendo, pero no limitando, la trasfusión de sangre con una razón fisiológica de componentes, fácil transporte y transfusión, menor volumen de anticoagulantes y aditivos trasfundidos al paciente, y menor exposición a donantes. La sangre total es una herramienta con un potencial reemergente que puede ser implementado en centros de trauma civil con óptimos resultados y menor demanda técnica.

Resumen Introducción: El ángulo QRS-T espacial es la diferencia entre el vector del QRS y la T. Se conoce el valor diagnóstico y pronóstico de este marcador en enfermedades cardíacas. Es prioritario determinar si estas mediciones son reproducibles con confiabilidad en nuestro medio. Métodos: En 30 adultos se obtuvieron electrocardiogramas, independientemente del diagnóstico. Las mediciones las realizaron dos estudiantes de Medicina de quinto año, un médico interno y un cardiólogo, posterior a un reentrenamiento sobre la medición del ángulo por el método de transformación visual. Con identificación cegada, ingresaron en un aplicativo web las mediciones del QRS y la T. El ángulo fue estimado a partir de la plantilla de Cortez y colaboradores. Sobre el 20% de las lecturas se estimó un acuerdo intraobservador y en el 100% de las lecturas un acuerdo interobservador. Los puntos de corte para estimar el acuerdo Kappa fueron < 105s (normal), 105° a 135° (limítrofe) y > 135° (anormal). Resultados: Se excluyó un ECG por calidad deficiente. El rango del ángulo estuvo entre 72.9° y 176.2°. El acuerdo interobservador entre los 4 evaluadores fue kappa = 0.786 (IC95% 0.728-0.823, p < 0.0001) y el intraobservador kappa = 1.0 (p 0.007). Conclusión: Es el primer estudio sobre el ángulo QRS-T espacial realizado en Colombia. Se logró un acuerdo inter- e intraobservador adecuado en la medición del ángulo por el método de transformación visual, en observadores con diferente experticia, con un kappa mayor de 0,75. Este resultado apoya la reproducibilidad de esta medición en estudios posteriores en Colombia.


Abstract Introduction: The spatial QRS-T angle is the difference between the QRS vector and T vector. The diagnostic and prognostic value of this marker in heart disease is well-known. It is essential to determine whether these measurements are reproducible and reliable in Colombia. Methods: Electrocardiograms (ECG) were obtained from 30 adults, regardless of the diagnosis. The measurements were made by two fifth-year medical students, a houseman/ intern, and a cardiologist. They were all re-trained on the measurement of the angle using the visual transformation method. With blinded identification, they entered the QRS and T measurements into a web application. The angle was estimated from the template of Cortez et al. About 20% of the readers showed an intra-observer agreement and an inter-observer agreement in 100%. The cut-off points to estimate the agreement Kappa were < 105° (normal), 105° a 135° (borderline), and >135° (abnormal). Results: One ECG was excluded due to poor quality. The range of the angle was between 72.9° and 176.2°. The inter-observer agreement between the 4 evaluators gave a kappa = 0.786 (95% CI; 0.728-0.823, P < .0001), and the intra-observer agreement a kappa = 1.0 (P = .007). Conclusion: This the first study on the spatial QRS-T angle performed in Colombia. A good inter- and intra-observer agreement was achieved in the measurement of the angle by the visual transformation method by observers with different levels of expertise, with a Kappa greater than 0.75. These results support the reproducibility of this measurement in subsequent studies in Colombia.

Introducción. El trauma es reconocido como una epidemia global que varía según las regiones donde se presenta. Una parte de la carga de la enfermedad está determinada por los años perdidos de vida potencial, la cual es una estimación de amplio uso para la vigilancia en salud pública; sin embargo, existe poca información disponible en relación a esto. El objetivo de este estudio fue determinar la tendencia de los años perdidos de vida potencial por el trauma en un período de nueve años.Métodos. La información se obtuvo del Instituto Nacional de Medicina Legal y Ciencias Forenses en un período de nueve años (2007-2015). La población de referencia se determinó con base en las proyecciones del Departamento Administrativo Nacional de Estadística, que indicaron una expectativa de vida de 75 años.Resultados. En el 2015, a las lesiones por traumatismos en Colombia les correspondieron 1.920,7 años perdidos de vida potencial por cada 100.000 personas. Durante el período de estudio, las principales causas fueron los homicidios (rango, 51 a 68 %) y los accidentes de tránsito (rango, 19 a 28 %); la relación entre hombres y mujeres fue de 7:1, y la tendencia estadística fue hacia la disminución de los años perdidos de vida potencial.Conclusiones. Los homicidios siguen aportando el mayor número de lesiones por trauma y años perdidos de vida potencial. Los hombres jóvenes continuaron siendo la población mayormente afectada. Se conceptúa la necesidad de incrementar los esfuerzos para mejorar la vigilancia en salud pública y ahondar en las intervenciones oportunas relacionadas con el trauma (AU)


Introduction: Trauma is a worldwide leading cause of external injuries that varies according to the regions. In 2015, trauma injuries were the third cause of Disability Adjusted Life Years (DALYs) with the 9% of the total global burden of disease. A portion of the burden of disease is determined by the Years of Potential Life Lost (YPLL). In Colombia in 2015, from the total of deaths due to external cause injuries, homicides had the highest number of YPLL with a total of 495.667; traffic accidents had 236.237 YPLL and accidental deaths 90.745 YPLL. The YPLL trauma trends and baselines are important to public health surveillance but there's no consolidated description. The aim of this study is to determine trauma trends in a five-year period.Material and methods: The information was obtained from the reports of Instituto Nacional de Medicinal Le-gal y Ciencias Forenses in a nine-year period (2007-2015). The reference population was identified through the population projections from the Departmento Administrativo Nacional de Estadística (DANE). YPLL calculation was compared to a 75 years life expectancy.Results: In 2015 trauma injuries in Colombia had a total of 1.920,7 YPLL per 100.000 people. Overall the study period, leading cause of YPLL was homicides (range= 51-68%) and traffic accidents (range= 19-28%), ratio male: female was 7:1 and YPLL observed had decreased. The percentage of the total decrease was 6.3%, the highest increase was observed in 2009 with a raise of 30.5%.Conclusions: Homicides are a major public health issue such as the leading cause in YPLL of trauma injuries. Despite there was no increase in sex ratio, younger males are getting more affected through the time increasing YPLL in this population group. More efforts are needed to improve public health surveillance for assessing baselines, DALYs, policies and evidence for interventions in trauma-related injuries (AU)

Abstract Aim: Our objective was to describe the variations in casualties admitted to the emergency department during the period of the negotiation of the comprehensive peace agreement in Colombia between 2011 and 2016. Methods: A retrospective study of all hostile military casualties managed at a regional Level I trauma center from January 2011 to December 2016. Patients were subsequently divided into two groups: those seen before the declaration of the process of peace truce (November 2012) and those after (negotiation period). Variables were compared with respect to periods Results: A total of 448 hostile casualties were registered. There was a gradual decline in the number of admissions to the emergency department during the negotiation period. The number of soldiers suffering blast and rifle injuries also decreased over this period. In 2012 there were nearly 150 hostile casualties' admissions to the ER. This number decreased to 84, 63, 32 and 6 in 2013, 2014, 2015 and 2016 respectively. Both, the proportion of patients with an ISS ≥9 and admitted to the intensive care unit were significantly higher in the period before peace negotiation. From August to December/2016 no admissions of war casualties were registered. Conclusion: We describe a series of soldiers wounded in combat that were admitted to the emergency department before and during the negotiation of the Colombian process of peace. Overall, we found a trend toward a decrease in the number of casualties admitted to the emergency department possibly in part, as a result of the period of peace negotiation.


Resumen Objetivo: Describir las variaciones en los soldados heridos en combate admitidos al departamento de emergencias durante el periodo de negociación del proceso de paz colombiano entre el 2011 y el 2016. Métodos: Estudio retrospectivo de todos los soldados heridos en combate que fueron manejados en un centro de trauma desde Enero del 2011 a Diciembre del 2016. Los pacientes se dividieron en dos grupos: aquellos que ingresaron al departamento de emergencias antes de la tregua del proceso de paz (Noviembre 2012) y aquellos que ingresaron durante la negociación. Los grupos se compararon con respecto a los periodos de tiempo. Resultados: Un total de 448 soldados heridos en combate fueron incluidos. Hubo una disminución gradual en el número de admisiones durante el periodo de negociación. Además, el número de soldados que sufrieron heridas por explosiones y fusiles disminuyó durante este periodo. En el 2012 se registraron 150 soldados heridos en combate. Este número disminuyó a 84, 63, 32 y 6 en los años 2013, 2014, 2015 y 2016 respectivamente. La proporción de pacientes con un ISS ≥9 y la proporción de admitidos a la unidad de cuidado intensivo fueron significativamente mayores en el periodo antes de la negociación. Desde Agosto a Diciembre/2016 no se registraron admisiones. Conclusión: Este estudio describe una disminución gradual en el número de soldados heridos en combate admitidos al departamento de emergencia en un periodo de 6 años. Este fenómeno pudo deberse al periodo de negociación del proceso de paz.

BACKGROUND: Trauma information systems are needed to improve decision making and to identify potential areas of intervention. OBJECTIVE: To describe the first year of experience with a trauma registry in two referral centers in southwest Colombia. METHODS: The study was performed in two referral centers in Cali. Patients with traumatic injuries seen between January 1 and December 31, 2012, were included. The collected information included demographics, mechanism of trauma, injury severity score (ISS), and mortality. A descriptive analysis was carried out. RESULTS: A total of 17,431 patients were registered, of which 67.8% were male with an average age of 30 (±20) years. Workplace injuries were the cause of emergency consultations in 28.2% of cases, and falls were the most common mechanism of trauma (37.3%). Patients with an ISS ≥15 were mostly found in the 18-35-year age range (6.4%). Most patients who suffered a gunshot wound presented an ISS ≥15. A total of 2.5% of all patients died, whereas the mortality rate was 54% among patients with an ISS ≥15 and a gunshot wound. CONCLUSION: Once the trauma registry was successfully implemented in two institutions in Cali, the primary causes of admission were identified as falls and workplace injuries. The most severely compromised patients were in the population range between 18 and 35 years of age. The highest mortality was caused by gunshot wounds. Abstract Background: Trauma information systems are needed to improve decision making and to identify potential areas of intervention. Objective: To describe the first year of experience with a trauma registry in two referral centers in southwest Colombia. Methods: The study was performed in two referral centers in Cali. BACKGROUND: Dynamic and efficient resuscitation strategies are now being implemented in severely injured hemodynamically unstable (HU) patients as blood products become readily and more immediately available in the trauma room. Our ability to maintain aggressive resuscitation schemes in HU patients allows us to complete diagnostic imaging studies before rushing patients to the operating room (OR). As the criteria for performing computed tomography (CT) scans in HU patients continue to evolve, we decided to compare the outcomes of immediate CT versus direct admission to the OR and/or angio suite in a retrospective study at a government-designated regional Level I trauma center in Cali, Colombia. METHODS: During a 2-year period (2012-2013), blunt and penetrating trauma patients (≥ 15 years) with an Injury Severity Score (ISS) greater than 15 who met criteria of hemodynamic instability (systolic blood pressure [SBP] <100 mm Hg and/or heart rate >100 beats/min and/or ≥ 4 U of packed red blood cells transfused in the trauma bay) were included. Isolated head trauma and patients who experienced a prehospital cardiac arrest were excluded. The main study outcome was mortality. RESULTS: We reviewed 171 patients. CT scans were performed in 80 HU patients (47%) immediately upon arrival (CT group); the remaining 91 patients (53%) went directly to the OR (63 laparotomies, 20 thoracotomies) and/or 8 (9%) to the angio suite (OA group). Of the CT group, 43 (54%) were managed nonoperatively, 37 (46%) underwent surgery (15 laparotomies, 3 thoracotomies), and 2 (5%) underwent angiography (CT OA subgroup). None of the mortalities in the CT group occurred in the CT suite or during their intrahospital transfers. CONCLUSION: There was no difference in mortality between the CT and OA groups in HU patients. CT scan was attainable in 47% of HU patients and avoided surgery in 54% of the cases. Furthermore, CT scan was helpful in deciding definitive/specific surgical management in 46% scanned HU patients who necessitated surgery after CT. Antecedentes: La prevalencia de los factores de riesgo tradicionales asociados a enfermedad coronaria ha cambiado con el tiempo. En la actualidad se ha aumentado la frecuencia de dislipidemia, hipertensión y diabetes mellitus, pero disminuido la de tabaquismo. El efecto de cohorte al nacer en sujetos con enfermedad coronaria como aproximación de la verdadera prevalencia en el tiempo no se ha estudiado en Latinoamérica. Objetivo: Determinar la tendencia temporal de la prevalencia de factores de riesgo tradicionales para enfermedad coronaria por efecto de cohorte al nacer en una población de alto riesgo. Métodos: En pacientes con enfermedad coronaria se estimó la prevalencia de: diabetes mellitus, tabaquismo, hipertensión y dislipidemia. Se define como la presencia del efecto de cohorte al nacer la metodología estadística, epidemiológica y sociológica que pretende identificar la influencia del entorno en el transcurso de la vida de acuerdo con la década de nacimiento. Se realizaron análisis ajustados por género. Resultados: Se incluyen 3,056 sujetos. De ellos, el 72% son varones, la mediana de edad fue de 61 años (rango intercuartílico = 53-69). Para toda la población de estudio, la prevalencia de hipertensión fue del 62.3%, diabetes mellitus del 48.8%, tabaquismo activo del 18.8% y dislipidemia del 48.8%. Para diabetes mellitus y dislipidemia se observó un incremento de la prevalencia en cada cohorte según la década de nacimiento, mientras que para la hipertensión se vio una disminución en las mismas décadas de nacimiento. Conclusiones: La prevalencia de los factores de riesgo tradicionales ha cambiado con el tiempo y se evidencia la presencia del efecto del momento al nacer, influido posiblemente por las condiciones sociales del entorno en cada década de la vida.

BACKGROUND: The traditional management of complex penetrating duodenal trauma (PDT) has been the use of elaborate temporizing and complex procedures such as the pyloric exclusion and duodenal diverticulization. We sought to determine whether a simplified surgical approach to the management of complex PDT injuries improves clinical outcome. METHODS: A retrospective review of all consecutive PDT from 2003 to 2012 was conducted. Patients were divided into three groups according to a simplified surgical algorithm devised following the local experience at a regional Level I trauma center. Postoperative duodenal leaks were drained externally either via traditional anterior drainage or via posterior "retroperitoneal laparostomy" as an alternate option. RESULTS: There were 44 consecutive patients with PDT, and 41 of them (93.2%) were from gunshot wounds. Seven patients were excluded owing to early intraoperative death secondary to associated devastating traumatic injuries. Of the remaining 36 patients, 7 (19.4%) were managed with single-stage primary duodenal repair with definitive abdominal wall fascial closure (PDR + NoDC group). Damage-control laparotomy was performed in 29 patients, (80.5%) in which primary repair was performed in 15 (51.7%) (PDR + DC group), and the duodenum was over sewn and left in discontinuity in 14 (48.3%). Duodenal reconstruction was performed after primary repair in 2 of 15 cases and after left in discontinuity in 13 of 14 cases (DR + DC group). The most common complication was the development of a duodenal fistula in 12 (33%) of 36 cases. These leaks were managed by traditional anterior drainage in 9 (75%) of 12 cases and posterior drainage by retroperitoneal laparostomy in 3 (25%) of 12 cases. The duodenal fistula closed spontaneously in 7 (58.3%) of 12 cases. The duodenum-related mortality rate was 2.8%, and the overall mortality rate was 11.1%. CONCLUSION: An application of basic damage-control techniques for PDT leads to improved survival and an acceptable incidence of complications. LEVEL OF EVIDENCE: Therapeutic study, level IV.

BACKGROUND: The objective of this study was to describe a comprehensive five-step surgical management approach for patients with penetrating liver trauma based on our collective institutional experience. METHODS: A prospective consecutive study of all penetrating liver traumas from January 2003 to December 2011 at a regional Level I trauma center in Cali, Colombia, was conducted. RESULTS: A total of 538 patients with penetrating thoracoabdominal trauma were operated on at our institution. Of these, 146 had penetrating liver injuries that satisfied the inclusion criteria for surgical intervention to manage their hepatic and/or associated injuries. Eighty-eight patients (60%) had an American Association for the Surgery of Trauma-Organ Injury Scale (AAST-OIS) of Grade III (54 patients, 37%), Grade IV (24 patients, 16%), and Grade V (10 patients, 7%). This group of patients required advanced "complex" techniques of hemostasis such as the Pringle maneuver (PM), perihepatic liver packing (PHLP), and/or hepatotomy with selective vessel ligation (SVL). The focus of our study was this subgroup of patients, which we further divided into two as follows: those who required only PM + PHLP (55 patients, 63%) to obtain control of their liver hemorrhage and those who required PM + PHLP + SVL (33 patients, 37%). Of the patients who required PM + PHLP + SVL, 10 (27%) required ligation of major intrahepatic branches, which included suprahepatic veins (n = 4), portal vein (n = 4), retrohepatic vena cava (n = 1), and hepatic artery (n = 1). The remaining 23 patients (73%) required direct vessel ligation of smaller intraparenchymal vessels. The overall mortality was 15.9% (14 of 88), with 71.4% (10 of 14) related to coagulopathy. Mortality rates for Grade III was 3.7% (2 of 54), for Grade IV was 20.8% (5 of 24), and for Grade V was 70% (7 of 10). The mortality in the PM + PHLP + SVL group was higher compared with the PM + PHLP group (12 [36.4%] vs. 2 [3.6%], p = 0.001]. CONCLUSION: For those patients who fail to respond to PM + PHLP and/or those who have AAST-OIS penetrating liver injuries, Grades IV and V would benefit from immediate intraparenchymal exploration and SVL.

Introducción. En el mundo se reconoce la importancia de los sistemas de registro de trauma. En Latinoamérica los sistemas disponibles de este tipo son poco confiables y no tienen continuidad. El del International Trauma System Development Program es el primero en implementarse en Colombia. Objetivo. Describir la experiencia de la implementación entre el 6 de enero y el 31 de julio de 2012 de un sistema de registro de trauma en dos hospitales de nivel III (equivalente al nivel I en complejidad de los hospitales en Estados Unidos) en Cali, Colombia. Método. El sistema de registro de trauma incluye información prehospitalaria y hospitalaria, y el estado al egreso del paciente, en tiempo real. Cada hospital tiene una estrategia para la captura electrónica de datos. Se presenta un análisis descriptivo y exploratorio de un ensayo piloto de siete meses. Resultados. Se registraron 9.080 pacientes en ambos hospitales. En el análisis global, el 67,1 % eran hombres; la edad promedio fue de 30 ± 20 años; el 68% eran mayores de 18 años. El 24,5 % de los pacientes fueron hospitalizados; de estos, 300 (13,5%) requirieron hospitalización en la unidad de cuidados intensivos. En cuanto a la gravedad del trauma, el 5,6 % de los pacientes tenía un puntaje de gravedad de las lesiones (Injury Severity Score, ISS) igual o mayor de 15. La tasa de mortalidad global fue del 3 %. Conclusión. Se logró la implementación del sistema de registro de trauma e identificar las causas de consulta, morbilidad y muerte por trauma. Esto contribuye al desarrollo de la investigación sobre pacientes con trauma, y a reorientar los servicios de urgencias y los del sistema regional de trauma y emergencias. Introduction: The importance of the Trauma Registry System is recognized worldwide. The available TRS in Latin America are unreliable and have no continuity. The International Trauma Registry of the Panamerican Trauma Society – International Trauma System Developing Program (ITSDP) is the first one to be implemented in Colombia. Objective: To describe the experience with the implementation of a TRS at two third level of care hospitals (equivalent to complexity level I in the United States) in Cali, Colombia, in the period January 6 to July 31, 2012. Method: TRS includes prehospital, hospital, and discharge status in real-time. Each hospital has its own electronic data capture strategy. A seven-month period pilot trial descriptive analysis is presented. Results: 9,080 patients were registered, 67,1% were male; mean age was 30,2 ± 20 years, 68,0% older than 18 years; 24.5% were hospitalized, of which 300 (13,5%) required ICU. Regarding severity of trauma, 5,6% of patients had ISS ≥ 15. Overall mortality rate was 3,0%. Conclusion: Implementation of the SRT was feasible and allowed the identification the cause of presentation, morbidity, and mortality rates. This improves research on trauma better direction of the emergency and trauma services.

BACKGROUND: Early damage-control resuscitation (DCR) indicators have not been clearly discerned in patients with penetrating abdominal trauma. Our objective was to identify these clinical indicators that could standardize a DCR initiation policy in this subset of patients. METHODS: Prospective data collection from January 2003 to October 2010 at a Level I trauma center in Cali, Colombia. All adult (>15 years) patients with abdominal gunshot wounds (GSWs) were included. They were divided into two groups: those who underwent DCR and those who did not. Both groups were compared by demographics, clinical variables, severity scores, and overall mortality. Other scores were compared with our newly devised model using the area under the receiver operating characteristic curve (AUROC). RESULTS: There was a total of 331 abdominal GSWs. Of these, a total of 162 (49%) underwent DCR. The overall mortality was 11.2%. Multivariate analysis identified (A) acidosis (base deficit ≥ 8); (B) blood loss (hemoperitoneum > 1,500 mL); (C) cold (temperature < 35 °C); (D) damage (New Injury Severity Score > 35) as significant clinical indicators that aided in the decision process of early implementation of DCR. The Trauma-Associated Severe Hemorrhage (AUROC, 0.8333), McLaughlin (AUROC, 0.8148), ABC (AUROC, 0.7372) scores and our ABCD mnemonic (AUROC, 0.8745) were all good predictors of DCR, and the difference between them was statistically significant (p < 0.001). CONCLUSION: We have identified (A) acidosis (base deficit ≥ 8); (B) blood loss (hemoperitoneum > 1,500 mL); (C) cold (temperature < 35 °C); (D) damage (New Injury Severity Score > 35) as significant clinical indicators that aided in the decision process of early implementation of DCR for patients with abdominal GSWs. RESUMO

OBJECTIVE: To describes the experience in the implementation of a TRS in two hospitals in Cali, Colombia. METHODS: The TRS includes prehospitalary, during hospitalization and discharging status information of each patient. Each hospital has an electronic data capture strategy. A three month Pilot-period descriptive analysis is presented. RESULTS: 3293 patients has been registered, 1626 (49.4%) from the Public hospital and 1613 (50.6%) from the Private one. 67.2% were men; the mean age ±SD was 30.5±20 years; 30.5% were less than 18 years. The overall mortality rate was 3.5%. The most frequent consulting cause were falls (33.7%); 11.6% of injuries are secondary to fire gunshot, and this group where mortality rate was 62%. CONCLUSION: It was determined the needing for the TRS implementation and the mechanisms to provide continuity. The registry becomes an information source for the investigation developing. It was identified the causes of consult, morbidity and death due to trauma that will allow a better planning of the emergency services and of the regional trauma system in order to optimize and reduce the attention costs. Based on optimal information system it will be able to present the necessary adjusts to redesign the Trauma and Emergencies Attention System in the Colombian South-West.
